The 2009/2010 season of the Treta Liga Istok was won by FK Borec. Borec finished the season with 63 points, 10 points ahead of second-placed FK Metalurg Skopje.

The bottom two teams, FK Belasica and FK Makedonija Gjorce Petrov, were relegated to the Cetvrta Liga.
